Medical Courier Privacy Addendum

This Medical Courier Privacy Addendum ("Addendum") supplements the general Privacy Policy of PSK Logistics, LLC("Company," "we," "us," or "our") and applies specifically to medical, laboratory, pharmaceutical, diagnostic, and other healthcare-related courier services.

This Addendum is intended to clarify our privacy, confidentiality, and data-handling practices when transporting materials associated with healthcare services.

1. Purpose and Scope

This Addendum applies to:

• Medical courier services
• Laboratory specimen transport
• Pharmaceutical deliveries
• Diagnostic materials
• Medical records or healthcare-related documents
• Any delivery involving healthcare providers, laboratories, clinics, hospitals, pharmacies, or diagnostic facilities

This Addendum governs only those services and does not replace or override our general Privacy Policy.

2. No Provision of Medical Care

PSK Logistics, LLC is a courier and logistics provider, not a healthcare provider. We do not diagnose, treat, or provide medical advice. Our role is limited strictly to the secure transport of items and documents as directed by our clients.

3. Protected Health Information (PHI)

3.1 Handling of PHI

• We do not request, collect, or intentionally store Protected Health Information (PHI) as defined under the Health Insurance Portability and Accountability Act (HIPAA).
• Any PHI that may be incidentally visible during the course of a delivery is treated as strictly confidential.
• We do not access, copy, photograph, disclose, or retain PHI beyond what is necessary to complete transport

3.2 Limited Data Exposure

Information provided to us is limited to:

• Pickup and delivery locations
• Authorized contact names
• Reference or tracking numbers
• Handling instructions (e.g., temperature control, biohazard labeling)

Clients are responsible for ensuring PHI is properly packaged and secured prior to pickup.

4. HIPAA Status and Business Associate Agreements

• Unless expressly agreed in writing, PSK Logistics, LLC does not act as a Business Associate under HIPAA.
• When required by law or contract, a separate Business Associate Agreement (BAA) will govern PHI obligations.
• In the absence of a BAA, our services are structured to minimize or avoid PHI exposure entirely.

5. Confidentiality Obligations

All employees, drivers, and independent contractors:

• Receive confidentiality and privacy training
• Are required to sign confidentiality or non-disclosure agreements
• Are instructed on secure handling and chain-of-custody procedures

Unauthorized disclosure of medical or healthcare-related information is grounds for immediate termination of engagement.

6. Security Safeguards

We maintain reasonable administrative, technical, and physical safeguards appropriate for medical courier services, including:

• Restricted access to shipment details
• Secure dispatch and routing systems
• Chain-of-custody documentation
• Incident reporting procedures

No electronic storage of medical records or test results is maintained by the Company unless contractually required.

7. Incident Response and Breach Notification

In the event of suspected loss, theft, or unauthorized disclosure of medical-related materials:

• We will promptly investigate the incident
• Notify the client in accordance with contractual obligations
• Cooperate with client compliance and regulatory requirements

The Company does not independently determine HIPAA breach notifications unless acting under a BAA.

8. Data Retention

Medical courier delivery records are retained only as necessary for:

• Proof of delivery
• Billing and audit purposes
• Insurance and regulatory compliance

We do not retain specimen data, test results, or patient records.

9. Client Responsibilities

Clients are responsible for:

• Proper labeling and packaging of medical materials
• Ensuring compliance with applicable healthcare regulations
• Limiting disclosure of PHI to the minimum necessary

The Company is not responsible for improperly packaged or labeled materials.

10. Compliance with Laws

We comply with applicable federal, state, and local laws governing medical courier services, including transportation and safety regulations. This Addendum does not create healthcare provider obligations where none legally exist.

11. Updates to This Addendum

This Addendum may be updated periodically. Any changes will be posted with an updated effective date and apply prospectively.
